Pet Adoption

The Adoption Process                             Our Pet List

When you have found a Countryside Animal Rescue (CAR) Animal that you are interest in adopting, please submit your application using the online method below or download our applications for mail or fax uses.

Onlin Forms...

Downloadable Forms...

Fill out the application completely and send it to us as directed on the application.. Your Application will than be evaluated, references checked and you will be contacted.

We are very serious about placing our animals in a good home which is a good match for both the adopter and the animal.

We will do a home visit at which time we will discuss what the animal will require and address any concerns you might have at that time.

If approved, an adoption contract will be developed. Once the contract has been signed and the fee paid you will take the animal to your home.

CAR will continue to monitor the animal from time-to-time to ensure the relationship between the adopter and the animal is continuing in a positive direction.

Adoption Policies

CAR asks that all prospective adopters fill out an extensive Adoption Application (.doc format), or Adoption Application (.pdf format), or Cat Adoption Application. Fill out the application completely and send it to us as directed on the forms. This is to ensure that every animal and family are a good and FOREVER match.

  • CAR asks that you supply the name and phone number of your veterinarian if you currently have a pet or pets.
  • CAR asks that you supply two personal references.
  • CAR asks that we be notified in the event that your animal is lost or stolen so we can assist in reuniting you with your pet.
  • CAR reserves the right to deny, stop the progress of, or reverse an adoption to anyone we suspect of using animals for any of the following purposes: Experimentation, cult rites, food, dog fighting, breeding, guard dogs, attack dogs, or for any other purposes contrary to the Mission Statement of CAR.
  • CAR reserves the right to deny adoption of “Bully Breed” dogs to families with small children.
  • CAR adoption fees include the animal being fully vetted. At the time of adoption the new owner is made aware of any known physical or temperament issues. Upon signing the adoption contract you are assuming full responsibility for the health and well-being of your new pet.
  • CAR will no longer be financially responsible for the animals that are adopted but will gladly help with advice, direction or educational needs that may arise.
  • CAR will follow up each adoption with periodic phone calls or schedule Adoption Home Visits to ensure that the animal and family are doing well and that this will be the animal’s forever home.
  • CAR believes that your pets should be accepted into your home as an extension of your family, therefore:
  • Cats will not be adopted as “barn cats or outside cats.” We believe the best way to keep a cat safe is to keep him or her indoors as a companion animal.
  • CAR will only adopt to individuals that are eighteen (21) years of age or older.
  • CAR will not adopt an animal as a gift. The animal must be adopted by the person or family that will be providing a home for the animal.
  • CAR will have a representative make a Home Visit with the potential adopting person or family. This will include an informal interview to make you aware of the responsibilities and to ensure that this is the best possible environment for the animal.
  • CAR requires that if other dogs or cats are currently in the home that all other pets are spayed or neutered and current on all vaccinations before any adoption may occur.

CAR works hard to find loving, permanent homes for all-breeds, including mixed-breeds, of unwanted or mistreated dogs & cats.
